Home and support services for older adults
We provide a number of services to help older people who need extra support to live independently in their own homes and the community.
Commonwealth Home Support Program
The Commonwealth Home Support Program (CHSP) connects older people with support services to help them continue to live independently and safely at home.
Who is eligible?
People aged 65 years and over, or 50 years and over for Aboriginal and Torres Strait Islander people who:
- are registered with My Aged Care,
- assessed as eligible for support, and
- live in the City of Yarra Council service area.
How do I register?
To access our CHSP senior support services you must be registered with My Aged Care, online at My Aged Care on by phone 1800 200 422.
Need an interpreter?
If you require an interpreter, My Aged Care can arrange one for you.
Is there a fee?
Most of our senior support services are provided for a fee, which varies based on the type of service you receive and your income.

Know your rights
When you receive services from us, you have the right to expect high-quality care and to be treated with respect and dignity. As a registered aged care provider for Category 1–3 services. We must meet specific responsibilities including the Code of Conduct and the Statement of Rights.
Senior Support Services
The following Senior Support Services are delivered as part of the Commonwealth Home Support Program (CHSP).
Food services
We deliver a three-course chilled meal to your home, which you can heat and eat at your convenience, along with some fresh fruit.
We cater for most dietary needs, including texture modified meals. We can also help you with heating your meal. More than just a meal, our food service provides important social connection. Enquire about food services online or contact Senior Support Services on 9205 5285.
Group social support
We offer a range of recreational activities, events and social support networks to help you stay active and engaged with your community.
Home maintenance and repairs
We can help improve the safety of your home by helping with minor repairs and maintenance tasks, such as changing smoke alarm batteries and lawn mowing.
Community transport
Our community bus service provides transportation to the Willowview Centre, organised activities and shopping trips.
