
An assessment is required for most Government funded services. Generally this assessment is conducted in your home by an Aged & Disability Services Assessment Officer. During the assessment you will be able to ask any questions and discuss your care needs.
At this home visit, the type, and amount of service you need will be negotiated with you. As far as possible all services will be provided at a time that suits you.
The services will then be written in a confidential care plan which is kept by Aged & Disability Services. (You will also be given a copy to keep).

Your care plan can be reviewed at any time with you and/or your carer. If we can’t provide the service you need we can refer you to an appropriate service.
