Assessment & eligibility

An assessment is required for most Government funded services. Generally, this assessment is conducted in your home by an Aged & Disability Services Assessment Officer. During the assessment, you will be able to ask any questions and discuss your care needs.

Assessment

At this home visit, the type and amount of service you need will be negotiated with you. As far as possible, all services will be provided at a time that suits you. The services will then be written into a Confidential Care plan, which is kept by Aged & Disability Services. You will be provided with a copy of your Care Plan.

The Care Plan provides details of the:

  • agreed services to be provided
  • agreed number of service hours and frequency
  • fees to be charged for each service being provided
  • name and contact number of the Assessment Officer
  • the date of the review visit.

Your care plan can be reviewed at any time with you and/or your carer. If we can’t provide the service you need, we can refer you to an appropriate service.

Eligibility

The Home and Community Care (HACC) Guidelines established by the State Government provide the eligibility criteria for HACC services. Your must be a resident of the City of Yarra to receive Yarra’s HACC services.  Services are provided on the basis of eligibility, as well as assessed and prioritised need. This means that although you may fall into one of the categories and meet the eligibility criteria, in some instances, you may not automatically receive services.

In summary, the following people are eligible for HACC services:

  • younger people with a disability
  • frail older people
  • carers of younger people and frail older people with a disability

Council has a Priority of Access policy, which ensures that people most in need of assistance will receive it. Some people have higher and more complex needs and may be assessed as a higher priority for services.
