Council can help connect residents to additional support services, if they require more specialised care.
If your care needs are of a more complex nature, you may be eligible to receive assistance through the Linkages Program.
The Linkages Program is a case management service which can arrange additional services for people whose needs cannot be entirely met by the usual care services provided by Council.
The case manager will help you and/or your carer arrange services that are individually tailored to your needs. There is a waiting list for entry to this program.
Treatment Response Assessment Aged Care (TRAAC) is a service for older people living in the community or residential care who are experiencing a crisis that could lead to them going to hospital unnecessarily. The TRAAC program provides alternatives to allow people to be supported where they live. TRAAC services include:
- nursing care
- personal care
- support for the carer
- respite care
- home care
- delivered meals
- occupational therapy
- speech pathology
- information and advocacy.
The service provides the help of a short-term case manager to help you coordinate the services you need. There is no waiting list for entry into this program.
