An Assessment Officer will meet with you to discuss your care needs and consult you on the types and quantity of services you will receive.
There is a fee charged for all Yarra City Council Aged & Disability Services. However, the fee will vary according to the type of services you receive and is based on your income.
The Assessment Officer will discuss this with you during the assessment visit.
Our home care service aims to maintain your living environment to a reasonable standard and keep you safe and secure in your own home.

Our homecare service is not a spring cleaning service.
(If you need more intensive household cleaning, you may need to engage a commercial cleaning service. You can discuss this with our Assessment Officer if you wish.)
We recognise that some people may need assistance with their personal care.
You can be confident that personal carers can help you with your needs and that all personal care tasks will be performed in a sensitive manner that respects your feelings and meets your needs.
All non-medical related personal care needs can be arranged through the Assessment Officers.
Medical or nursing tasks can usually be arranged through your doctor or local nursing service. If you are unsure, the Assessment Officers will provide you with advice and information and can arrange a referral if required.

We recognise the valuable and important role of carers and understand that carers also need support and the occasional break. Yarra’s respite care service offers both in-house and centre-based services to give carers a break from their caring role.
Our in-house respite care service is provided in your home. These sessions usually occur at a pre-arranged time although they can sometimes be arranged on an ad hoc or emergency basis.
Feel comfortable discussing your needs with us, including the length of respite care and the type of care that will suit your circumstances.
Our home maintenance service is provided to maintain your living environment in a safe and secure manner.
As a guide, our home maintenance service includes:
The service is limited to tasks that would usually be done by a semi-skilled tradesperson. The service is unable to provide aesthetic work such as plastering, painting, or general gardening.
Works requiring major renovations or skilled trades are usually beyond the scope of the service. In these situations, we will provide information about the type of suitable tradespeople you need.
You will be charged for any materials purchased, and you will have to contribute to the cost of rubbish removal.
Food Services are available to those who are nutritionally at risk and unable to prepare regular meals as a result of frailty, illness or disability.
We will attempt to assist you meet your nutritional requirements in a manner that suits you.
Our food services options include:
Delivered Meals
A 3-course meal delivered (chilled) to your home, for you to heat at your convenience. If required, we can also help you heat the meal. In special circumstances meals can be delivered hot. Meals are delivered Monday to Friday with extra chilled meals delivered on Friday for the weekend.
Centre-based Meals
A lunchtime meal is available Monday to Friday at the Collingwood, Richmond and Carlton Senior Citizen’s Centres.
In Home Preparations
In-home meal preparation may be arranged as part of a personal care package (See Personal Care).
Shopping Assistance
For those people who require assistance with shopping, a weekly ‘door to door’ shopping trip is made to local shopping centres using Council’s community bus. (see Community Transport).
There is a range of other community meal options available within Yarra that are not directly provided by the City of Yarra.
Our staff can give you information about these services if needed.
Did you knowWe understand that opportunities for social and recreational activities may be important to your well-being.
There are many recreational and social activities operating across Yarra which welcome older people and people with disabilities. They include many local clubs which have particular interests such as gentle exercise, walking groups, arts and crafts, educational groups and many other activities.
These activities are provided in Neighbourhood Houses, Health Centres and community venues close to you.
Some social and recreational services can be arranged in your home through friendly visiting services. These services can match you with people of similar interests who are able to visit you on a regular basis and provide mutual support for each other. We can put you in contact with these services.
There may be a cost associated with some activities.
The community transport service provides assistance to frail older people and people with disabilities to access a wide range of community and social activities.
The service also provides transport for people attending Senior Citizen Centres, Willowview Day Centre and outings for these groups.
Shopping Trips
We also run regular ‘door to door’ shopping trips where you will be collected from your home and taken to the local shopping centre. You must be able to do your own shopping but we will help you on and off the bus, as well as loading and unloading your shopping bags or jeeps.
Importantly, support will be available in case of an emergency.
Willowview Day Centre offers a range of programs for frail older people and older people with disabilities. It also supports family members in their role as carers.
The Centre provides a warm, friendly and caring environment where people can socialise and participate in a range of enjoyable and interesting activities. A hot meal is provided for lunch and transport can be arranged.

The Willowview Day Centre is located next to the Collingwood Town Hall.
If your care needs are of a more complex nature, you may be eligible to receive assistance through the Linkages Program.
The Linkages Program is a case management service which can arrange additional services for people whose needs cannot be entirely met by the usual level of care services.
Linkages recipients are assigned a case manager, who can help you and/or your carer, arrange a package of services individually tailored to your needs.
Treatment Response Assessment Aged Care (TRAAC) is a service for older people living in the community or residential care who are experiencing a crisis that could lead to them going to hospital unnecessarily. The TRAAC program provides alternatives to allow people to be supported where they live. TRAAC services may include:
